Why should you not delay the decision of Hysterectomy? When should you take the decision of Hysterectomy right away?

There are two very important clinical conditions of uterus which demand attention

1) Uterine fibroids

2) Adenomyosis of uterus

Both these diseases are insidious in nature and develop over the years.

In the initial stages they are silent in nature but as time goes by, as the number of fibroids increase or as Adenomyosis grows in size, then the woman starts getting problems.

She may get manses very frequently. The flow during menses may be quite heavy and prolonged. Her periods may become painful. She may also complain of pain in lower abdomen at times.

When the menstrual pattern is so disturbed it is but natural that she will become irritable and guess who will become the target of her anger-her kids and her husband. It will disrupt the harmony of the home and will also affect the studies of children.

Medical complications will also come-she will develop Anemia which will lead to

-Weakness

-Low immunity

-Recurrent infections

Very big Fibroids also put pressure on surrounding structures. Some Fibroids grow so much that they can be felt in per abdomen examination and their height may come up to level of umbilicus.

Anteriorly they put pressure on Urinary Bladder and will lead to urinary retention, frequency, UTI-infections.

Laterally , they put pressure on Ureter and will produce back flow pressure changes in kidney.It will damage kidney functions that may ultimately become irreversible.

Posteriorly, they put pressure on Rectum and will give symptoms of chronic constipation.

All these symptoms strongly suggest that the decision of Hysterectomy should not be delayed.

Completed family is one more indication that the decision of Hysterectomy should not be delayed.

Suppose, we took decision of only-Myomectomy-removal of fibroids while leaving the uterus in situ, or we resected only the Adenmyosis from the uterus but the disease producing organ which is the uterus itself is still there.

In due course of time, this uterus will again produce the same disease that will require repeat major surgery and that surgery is going to be high risk.
