Real Women-Real Experiences-13.10.19- Lungs TB (Tuberculosis),Oligohydramnios & True Knot of Umbilical Cord, complicating a pregnancy

A 31 year old female,an IT professional, resident of Kalamboli, Panvel consulted us 1 year back for irregular menses.

She was put on treatment.

At that time her AKT- Anti Koch's Treatment for TB- Tuberculosis of lungs was in final stages.

In a few weeks she came back with missed periods and her Urine pregnancy test came positive.
She was counselled to keep her pregnancy.

The early pregnancy had many episodes of bleeding PV, per vaginum for which she required hospitalization for a couple of times.

Mid/second trimester was uneventful.

In early third/ last trimester she developed Oligohydramnios ( deficiency of water around fetus).

She partially responded to treatment and after some time water stopped to increase further

At one point she even complained of diminished fetal movements.

Fetal weight was good.

Upon approaching maturity, decision to deliver her by Elective Cesarean -LSCS was taken.

Our biggest concern had been that as she has had Tuberculosis, we might encounter intra-abdominal adhesions.

We had done a special sonography to check that and it was found to be normal.

We took extra precaution of keeping a General Surgeon as stand by in case a difficulty is encountered.

We noticed a true knot in the umbilical cord which could have been the reason for decreased blood supply to the baby and decreased fetal movements.

Rest of her operation was successful and uneventful.

She is a proud mother now.
