Real Women- Real Experiences-A difficult laparoscopic hysterectomy
A laparoscopic hysterectomy was posted for a 45 years old housewife, resident of Road Pali, Panvel.
Her complain was of Ademomyosis, Dysmenorrohea, Metrorrhagia and Menorrhagia.
She had had one delivery by Elective LSCS- Caesarean delivery 19 years back.
While doing a laparoscopic hysterectomy an important step is uterine manipulation by a stick like device called uterine manipulator which is put inside the uterus by a vaginal assistant.
The importance of the manipulation is to pull the uterus to contralateral(opposite) side while operating on one side. It provides traction as well as necessary protection to important structure like ureter while operating.
In this woman, the uterine manipulator could not be put as the cervical os was pin point and did not yield to dilatation.
The reason could have been fibrosis of cervix over the years, non dilatation of cervix because of elective LSCS
Our attempts to put Hysteroscope also failed.
Ultimately improvised/alternative techniques were used.
Additional port was taken and a special instrument 'Tenaculum' was introduced, which has two strong sturdy teeth to hold uterus.
Uterine manipulation was done laparoscopically and subsequent surgery was uneventful.
