Real Women- Real Experiences- 9.2.19-Small Ovarian Dermoid Cyst- Posing Treatment Dilemma
A 32 year old woman resident of Seawoods, Navi Mumbai, has been coming to OPD as she is desirous of second child. She has a 7 year old girl by normal delivery.
As part of fertility investigations, a sonography of pelvis was done.
A small echogenic ovarian mass of around 1.5x1.2x1.1 cm has been showing in the sonography.
A query diagnosis of Dermoid cyst has been kept by the radiologist.
The fact about Dermoid Cyst of ovary is, that they will keep on increasing in size. They contain totipotent cells and are capable of producing any kind of tissue-Epidermal,Mesodermal,Endodermal.
Most importantly, as they grow, they destroy the surrounding ovarian stroma.
Dermoid Cysts, need to be removed as soon as they are diagnosed, especially in a female who desires a pregnancy and ovarian function is important for her.
After proper pre- operative work up, she was taken in for Laparoscopy.
When we entered the abdominal cavity and examined the said ovary, it was looking, normal, with no obvious mass seen from outside .
We decided to make a nick and explore the ovarian stroma. Upon doing that, a small ovarian cyst , well encapsulated was found and removed.
We took it out, and on cutting it open- cheesy material- classical sign of Ovarian Dermoid Cyst came out.
The cyst was sent for histopathology.
Had it not been for the bold decision of exploring , an innocuous looking ovary would have kept on harboring the lesion which would have been detrimental for the patient , later on.
